    Do I need a referral prior to scheduling an appointment ?

    Prior to scheduling, please contact your insurance company to determine if a referral is required. This information may also be listed on your insurance card. It is the patient's responsibility to obtain the necessary referral. Failure to obtain referral can result in insurance not covering the visit.

    What should I bring to my appointment?

    Please bring a complete list of medications, including vitamins, herbal supplements or over-the-counter medications. Please bring your insurance card and copay. If you are a new patient, please arrive 10 minutes early to complete paperwork. Please bring a copy of immunizations, medical records and a copy of pertinent test and X-ray results to facilitate your visit.

    How do I obtain a refill on my prescriptions?

    Please contact your pharmacy to refill current prescriptions. Allow two working days for refills. The on-call physician after hours cannot refill medications.

    When are allergy shots given?

    Monday 10 a.m. - 11:30 a.m. 2 p.m. - 5 p.m.
    Tuesday NO SHOTS 2 p.m. - 5 p.m.
    Wednesday NO SHOTS 3 p.m. - 5 p.m.
    Thursday 8 a.m. - 11:30 a.m. 2 p.m. - 4:30 p.m.
    Friday 7:30 a.m. - 11:30 a.m. NO SHOTS

    If I need labs before my visit, how do I obtain the paperwork to take to the lab?

    Please call our office 24 hours in advance to request the paperwork.

    When can I expect notification of my test results?

    You should receive notification of test results by phone. In some cases, your physician may instruct you to call our office to obtain results or schedule a follow up visit.

    On which holidays is your office closed?

    Our office is closed on New Year's Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.
